If they reach exactly zero HP with no ongoing reduction (on fire, poison, etc.) I leave them as unconscious. If their friends win the fight such that the area is safe, or they are able to flee carrying the unconscious character, they can heal them. If there is an ongoing drain, death comes at -level in HP.
